So sad: Demi Lovato's ex Wilmer Valderrama, 38, is reportedly "completely devastated" over her apparent overdose on Tuesday.

The 25-year-old singer-songwriter, who is now on the third leg of her Tell Me You Love Me world tour, was reportedly taken to a Los Angeles hospital after being found unconscious at her home on Tuesday, according to NBC.

The LAPD confirmed to People.com that authorities "responded to a medical emergency at the 8000 block of Laurel View Dr.at 11:40 a.m" and the Los Angeles Fire Department said they transported a 25-year-old female patient to the hospital.

The 25-year-old star was taken from her Hollywood Home to receive treatment but her condition is now unknown.

Demi outlined her struggles with substance abuse in a new song released in June, entitled Sober. She relapsed after she left the treatment center, then entered a sober living facility for a year. "Praying for her right now, addiction is a terrifying disease".

Earlier, Dunn had urged her followers to "PLEASE PLEASE pray for my sweet niece DEMI LOVATO".

"I knew at a young age that some of my behaviours were a problem", she said, but added: "I wasn't in a place where I could quit by myself".

Her next concert was slated to be at Atlantic City Beach in Atlantic City, New Jersey, on Thursday night.

In the past, Lovato has spoken candidly in various interviews about her drug use and addiction to alcohol, as well as her struggle with an eating disorder.

Following her stint in rehab the star revealed she had been diagnosed as bipolar and had regularly taken cocaine.

Lovato's role as aspiring singer Mitchie Torres in the Disney TV movie "Camp Rock" introduced the world to her musical ability.

The star received huge support on Twitter from a number of celebrities, who expressed their sadness at the incident.

She had just performed in California on Sunday.

"Poor attractive spirit", singer Lily Allen said. In May, she tweeted that new music and lyrics would reveal the truth.
