Sanchez had been diagnosed with cardio myopathy, an incurable disease that causes heart failure, and for weeks she was in a Chicago hospital on a waiting list to get a transplant that would replace her heart, which didn't pump properly, with a donor heart.

The 11-year-old girl who received a surprise visit from Drake for her birthday during her hospitalization for heart surgery is going to receive an even greater gift: A new heart.

This past Sunday, she found out her second wish will be granted in an emotional moment captured on video.

First, the little girl wanted to meet the singer Drake, then she wanted a life-saving heart transplant. "Please say your prayers for Sofia and her doctors that everything goes smoothly".

"Sofia is getting a heart!" said the family through an update on the page, announcing that Sanchez would finally receive a transplant today.

Drake, who was in town for his concert at the United Center, shocked Sofia when he walked into her room at Ann & Robert H. Lurie Children's Hospital of Chicago.

Her mother, voice breaking tells her: "Well guess what?"

Sofia received the good news August 26. "Oh my god!" as she is overcome with emotion.

"You asked me to come, I'm here".

Drake's representatives contacted members of the Sanchez family shortly after Sofia made the video asking the rapper to visit her for her birthday.

"She was very happy yet nervous, but nervousness is common", Berger said.

Berger said Sanchez was in good spirits when she went into surgery this morning. Recovery is expected to take six to eight weeks, according to a post on a GoFundMe page set up for the Sofia.
